Eligibility Requirement

• are at least 16 years of age.
• have worked for Chuze Fitness for at least 12 consecutive months, AND  
• have worked at least 750 hours in their first 12 months OR in a calendar year. (Please note eligibility is not guaranteed until all hours are reviewed, you will be notified IF you become eligible) 

Enrollment Details

• Employees will be auto-enrolled in Chuze’s 401k plan starting at a 5% deferral rate. 
• The deferral rate earnings will be deducted from every pay period.
