Eligibility Requirements:
Full Time
- Are at least 16 years of age.
- Have worked for Chuze Fitness for at least 30 consecutive days
Part Time
- Are at least 16 years of age.
- Have worked for Chuze Fitness for at least 12 consecutive months, AND
- Have worked at least 500 hours in their first 12 months OR in a calendar year. (Please note eligibility is not guaranteed until all hours are reviewed, you will be notified IF you become eligible)
Enrollment Details:
- Employees will be auto-enrolled in Chuze’s 401k plan at a 5% deferral rate.
- You will have 30 days to opt-out or adjust your deferral rate
- The deferral rate earnings will be deducted every pay period
- Enrollment details will be sent by AON Pooled Employer Plan (Aon PEP)
401K POLICY DOCUMENTS
- Plan Overview – Highlights the features of the plan, such as eligibility, limits, and how to change your contribution rate.
- Fee Disclosure – Outlines all the fees associated with the plan, including administrative fees, investment fees, and any other charges.
- Automatic Contribution Notice – Standard required notice informing employees of the auto-enrollment feature of the plan. This will not override any current opt out elections for employees who have already opted out of the plan.
- QDIA Notice – Notice that if a participant does not elect their investment choice, the plan will put their money in the plan’s default fund – the Target Retirement Fund.
- Privacy Notice – A heads-up about how personal information within the plan will be handled and privacy protections.
