2021 Benefit Information

Eligible employees are notified of their opportunity to enroll in benefits via ADP, email, and text messaging during their enrollment period.  The initial enrollment periods occur shortly after an employee is hired or promoted to Full-time.  After the initial enrollment period, all eligible employees will have the opportunity to enroll in benefits during the annual Open Enrollment.  During each enrollment period, eligible employees are notified of their opportunity to enroll in benefits via ADP, email, and text messaging.  Participation is required as benefits do not roll over year to year.

Chuze offers a variety of benefits from insurance (medical, dental, vision, hospital and accident, life, and short term disability), to paid vacation time, to a 401(k).

Many benefit offerings are dependent on an employee’s full-time status (ave. 30+ hours worked per week).  You can check your official position status in the ADP employee portal in your Employment Profile.

Eligibility requirements for the 401(k) plan are different and can be found in the plan documents.  For a quick reference, here is a summary of the requirements. You must:

  • have completed at least 1 year of service
  • have worked at least 1,000 hours in your first year of employment or during a calendar year
  • be over the age of 21

Eligible employees are allowed to make benefits election and changes only during certain times of the year:

  • Within 30 days of your hire date as a full-time employee
  • Within 30 days of your status change to full-time
  • During the annual Open Enrollment period
  • Within 30 days of a qualifying life or family status event (applies only to certain changes)

Life or family changes sometimes require you to change your insurance elections outside of Open Enrollment.  

Following IRS regulations, you can make changes to your benefits that are consistent with your life or family status change within 30 days of the date that the status change occurred. If you miss the 30-day enrollment period, you must wait until the next Open Enrollment to make changes.  Life or Family changes do not allow you to change your benefit option or carrier (e.g. HMO to PPO) but rather the level of coverage (e.g., employee only coverage to a spouse or family coverage). 

Examples of Qualifying Life or Family Status Changes:

  • Change in position status (part-time to full-time, or full-time to part-time)
  • Loss of existing coverage elsewhere
  • Losing eligibility for Medicare, Medicaid, CHIP
  • Turning 26 and losing coverage through parent’s plan
  • Getting married or divorced
  • Having a baby or adopting a child
  • Death in the family
  • Moving to a different ZIP code or county where network coverage is not available

 You may be required to provide documentation to verify a Life or Family change.

Eligible employees can enroll in insurance benefits online via ADP within 30 days of their hire date or status change date.  A Benefit Guide will be emailed to them before their enrollment period and will include plan information as well as enrollment instructions.
The Chuze 401(k) plan is set up as an auto-enrollment plan.  Once an employee meets all of the eligibility requirements, they will be automatically enrolled into the plan at 3%, effective the beginning of the next quarter, or they can select their own contribution amount.  Eligible employees who do not want to be auto-enrolled will be given the opportunity to opt out.  Once you are a participant, you can change your contribution amount up to 4 times a year.  Changes will be effective the beginning of the following quarter.

Insurance plans selected during your New Hire window will be effective 1st of the month following 60 days of employment.

Selections made during Open Enrollment are effective on September 1st of that plan year. 

Coverage effective dates for mid-year entries due to Qualifying Events vary.

Payments will be deducted automatically from your pay check.

If you are enrolling in the DHMO plan, you will need to select a dentist as your regular provider. Follow the steps below to find your dentists/office number to email it to the Benefits Department.

Finding a Dentist:
  • Go to  Cigna.com
  • Click on “Find a Doctor, Dentist or Facility” at the top of the screen.
  • Select “Employer or School.”
  • Enter your Address or Zip Code
  • Click on Doctor by Type and select General Dentist.
  • Click Continue and choose the type of plan you are searching for.
    • DHMO – select “Cigna Dental Care Access Plus.”
    • Once you find an in-network dentist you like, make note of the 6-digit DHMO Office #. Email [email protected] with the Office #/PCP ID
