WELCOME TO YOUR OPEN ENROLLMENT INFO CENTER!

Open Enrollment is your yearly opportunity to change your benefit elections, add or drop dependents, and elect voluntary coverages. You have until the end of day, July 23rd, to login and enroll, or waive coverage.

As a reminder, this years open enrollment is “ACTIVE” — you must take action and ENROLL in benefits during the open enrollment window. Benefits will not roll over. If you are enrolled in benefits now and  you take no action, your benefits will be terminated 8/31/21.

All plans and benefit changes are effective September 1, 2021.

  • Per the Affordable Care Act (ACA) guidelines, to meet eligibility, you are required to be a Full-Time employee working a minimum of 130 hours per month (average of 30 hours per week).
  • Full Time status must be reflected in ADP
  • Full-Time employees can enroll the following eligible dependents:
    • Children up to age 26. Children include: Biological and adopted children (including those placed in your home for adoption)
    • Stepchildren and domestic partner children
    • Children for whom you are responsible to provide health coverage under a qualified medical child support order
  • Employment start date must be on or prior to 7/4/2021

Open Enrollment dates are:  July 19 – July 23, 2021

  • Enrollments will be reviewed and a confirmation will be emailed.
  • Benefits will become effective on September 1, 2021.
  • Your first payroll deduction for cost of coverage (premiums) will be deducted on September 3, 2021.

Plan Upgrades.

We are excited to announce that we have some great updates to our medical and vision plans for the upcoming plan year. For medical we have significantly LOWER out-of-pocket maximums, ZERO dollars for Telehealth, and $15 co-pays for BOTH plan options. This year’s vision plan has a $300 frame allowance that can now be used every 12 months (instead of 24 months) and the materials copay has been decreased from $20 to $10! With those changes and the increase of health care, there will be a slight rise in premium cost.

Chuze Fitness will continue to cover over 70% of the cost of the 3000 medical plan for ALL coverage levels and to provide life insurance for all full-time* employees. See your 2021-2022 Benefits Guide for information on each of the plans available to you and your family.

When you log into ADP the week of July 19, a pop-up window will appear to start your benefits enrollment. 

You will be able to access this enrollment window 24/7 from July 19th to July 23th.

Life Insurance

Chuze automatically enrolls all full-time employees in a Chuze-paid Life Insurance plan equalling 1x’s salary, with a minimum of $50,000 and maximum of $200,000. 

If you are interested in additional Life Insurance coverage for yourself and your family, you may also enroll in a voluntary Life Insurance plan. Chuze’s voluntary Life Insurance plan makes it easy for you to select the level of coverage that’s necessary for you and yours and has a high guaranteed coverage amount. This means that if you select a Life Insurance plan under the “guaranteed coverage amount”, you will be enrolled in the plan immediately and there will be no extra steps required. If you select a Life Insurance plan for an amount that is above the “guaranteed coverage amount”, you will be required to complete an Evidence of Insurability (EOI) before you may be fully enrolled in the plan. There is a chance that coverage requests over the guaranteed amounts may not be approved.

The Guaranteed Coverage Amounts:

  • Employee: The lesser of 3 times salary or $200,000
  • Spouse*: $25,000
  • Child*: $10,000

*In order for you to enroll your spouse and child(ren) in a voluntary Life Insurance plan, you must first enroll yourself.
